Partners at Work in the Community
After struggling with homelessness for over a decade, as well as living with high levels of anxiety and being wheelchair bound, 70-year-old Tammy Santos found relief through Partners in Care Foundation’s Assisted Living Waiver (ALW) that makes affordable housing available to eligible seniors and persons with disabilities in California. The ALW program placed her in a one-bedroom residency and hired a team to assist Tammy move her personal belongings from a storage facility to her new place. This move eliminated the ongoing monthly storage fee that has impacted her fixed income for the past 5 years. Care coordination services supported her with activities of daily living, like using the bathroom, and provided her with meals and skilled nursing services as needed.
Tammy hasn’t had a place to call home in the past 10 years, and the ALW program was the rainbow she was fortunate enough to find after living woefully under dark clouds. She now has the resources and care she needs to live happily and safely on her own.
