In 2013, Partners pioneered and operationalized the
nation’s first Community Care Hub, establishing a scalable
model that is helping shape the adoption of Community
Care Hubs across the country.
As healthcare continues to shift toward whole-person
care, Partners’ Hub serves as a Regional Community
Delivery System that unites healthcare organizations,
community-based organizations, and public agencies
through coordinated infrastructure and service delivery,
and reimbursable care pathways that support Medi-Cal,
Medicare, and dual-eligible populations.
By bridging healthcare and community-based services,
Partners connects individuals to the non-clinical supports
and services that influence health outcomes, helping older
adults and individuals with complex care needs heal, live
independently, and thrive while enabling healthcare and
community-based organizations to work together as one

What Is a Community Care Hub?
A Community Care Hub is a community-centered, organized delivery system that enables healthcare organizations to access, coordinate, and manage community-based services through a single infrastructure.
Rather than negotiating and managing multiple contracts with individual community-based organizations, healthcare partners connect to a coordinated network supported by centralized contracting, reimbursement, quality oversight, compliance, data exchange, reporting, and operational support.
By reducing administrative complexity and creating a single point of access, Community Care Hubs make it easier for health plans, health systems, and provider organizations to integrate community-based services at scale.
