Designing the Systems That Power Community-Based Care
For more than 28 years, Partners in Care Foundation has been a systems leader in community-based care, designing, leading, and scaling models of care that meet people where health is shaped every day.
Based in California’s San Fernando Valley and serving communities statewide, Partners is a nonprofit organization that unites healthcare and community-based services to help individuals access the right care, at the right time, in the right place. Through innovative programs and a No Wrong Door approach to care, we connect older adults and individuals across Medicare, Medi-Cal, and dual-eligible populations through reimbursable care pathways to the services and supports they need to live safely, heal fully, and thrive.
Our work spans care management, care transitions, medication safety, workforce development, and community-based service delivery. What distinguishes Partners is our ability to design and operationalize the systems that make community-based care work at scale.
As pioneers of the Community Care Hub model, Partners created a statewide framework that unites healthcare organizations, government agencies, and community-based organizations to deliver coordinated, whole-person care. What began as a California innovation is now being adopted across the country, helping shape how healthcare and community-based services work together to improve outcomes.
Today, Partners continues to advance the field through innovation and leadership at both the state and national levels, helping strengthen the systems that connect healthcare and community-based services.
Quality & Accountability
Partners offers an NCQA accredited scalable and community-rooted model that advances outcomes for California’s most medically and socially complex individuals. Leveraging our Community Care Hub, ECM, CICM, and LTSS integration expertise, we support health plans in delivering whole-person care that improves STARs, strengthens HEDIS performance, and enhances the lived experience of members throughout the state.

Partnership to Align Community Care
June Simmons, President and CEO of Partners in Care Foundation, serves as Co-Chair of the Partnership to Align Community Care, a national learning and action network focused on strengthening alignment between healthcare and community-based organizations.
Los Angeles Regional Coordinating Council on Aging & Disability
The Los Angeles Regional Coordinating Council on Aging & Disability is a regional convening focused on fostering a more coordinated, equitable, and effective system that enhances quality of life, particularly for older adults and individuals living with disabilities. The Council brings together public and private partners across healthcare, public health, and social care to align resources and strengthen coordination across systems.
Partners co-leads the Council’s convening, supporting collaboration among stakeholders to advance California’s “No Wrong Door” access systems and strengthen coordination with local and state public health departments.

Geriatric Social Work Education Consortium
GSWEC is a statewide collaborative focused on strengthening education and workforce preparation for social workers serving older adults and individuals with complex needs. GSWEC brings together academic institutions, healthcare organizations, and community-based providers to support effective, community-centered care.
Partners is actively engaged in GSWEC’s work, contributing practice-informed perspective grounded in real-world service delivery.
California Health Innovation Center℠
The California Health Innovation Center℠ (CHIC℠) is an innovation and partnership platform of Partners in Care Foundation. CHIC℠ supports the development, testing, and implementation of health improvement programs and services that advance community-based care and promote better health outcomes.
Through CHIC℠, Partners builds and manages strategic partnerships, including collaborations with for-profit organizations, to design and scale evidence-informed health promotion and chronic disease self-management initiatives. These programs are actively used across Partners’ Community Wellness efforts and are focused on helping individuals build the knowledge and skills needed to manage health risks and ongoing conditions.
CHIC℠ is also home to the California Health Promotion Initiative, which brings together local and statewide partners to expand access to education-based programs that improve quality of life and support long-term health and well-being for Californians.
