Good health is about more than managing a condition. It is about having the knowledge, confidence, and support to stay active, maintain independence, and continue doing the things you enjoy.
Partners in Care Foundation offers evidence-based wellness programs that help older adults improve their health, prevent falls, manage ongoing health conditions, and stay connected to their communities.
Through federal grant funding, Partners established the California Healthier Living Coalition, a statewide network dedicated to expanding access to proven wellness programs for older Adults. Recognized by the National Council on Aging as California’s statewide falls prevention coalition, the coalition helps organizations across the state share best practices, resources, and tools that support healthy aging and improve quality of life.

Our wellness programs are designed for older adults who want to stay active, improve their health, prevent falls, and continue living independently.
Whether you are managing a chronic health condition or simply looking for new ways to stay healthy, active, and connected, there is a program for you.
Classes are offered in English and Spanish and are available online, in person, and by telephone. You’ll learn practical skills, meet new people, and have fun while supporting your health and well-being.
Chronic Disease Prevention & Management Programs
Living with diabetes, heart disease, arthritis, chronic pain, depression, or another ongoing health condition? Our evidence-based workshops can help you feel better, stay active, and build the skills and confidence to manage your health.
Programs are offered in English and Spanish and meet once a week for six weeks. Workshops are available online, in person, and by telephone.

Healthier Living
Learn practical ways to feel better and manage chronic health conditions through healthy eating, physical activity, stress management, and relaxation techniques. Whether you’re living with diabetes, arthritis, heart disease, high blood pressure, chronic lung disease, cancer, or another ongoing health condition, this workshop can help.

Diabetes Self-Management
Gain tools and support to better manage diabetes, make healthy lifestyle choices, and improve your overall well-being.
Chronic Pain Self-Management
Learn strategies to manage chronic pain, stay active, and continue doing the things that matter most to you.
Fall Prevention Programs
The more confident you feel moving through your day, the more active and independent you can be.
Our evidence-based programs help you improve your balance, build strength, reduce your risk of falls, overcome the fear of falling, and stay active so you can continue enjoying the activities you love.
Programs are available in English and Spanish. Most workshops are offered online or in person.

A Matter of Balance
If you are worried about falling or have become less active because of that fear, this class can help.
You’ll learn practical strategies to make your home safer, reduce your risk of falls, manage your fear of falling, set realistic goals, and improve your strength and balance so you can stay active and continue enjoying the activities you love.
2-hour sessions once a week for 8 weeks or twice a week for 4 weeks
Bingocize
Have fun while improving your health. This fun, interactive workshop combines bingo, gentle exercises, and health education to help improve balance, coordination, and mobility.
1-hour sessions twice a week for 10 weeks
Tai Chi for Arthritis
Learn gentle Tai Chi movements that can help improve balance, flexibility, strength, and confidence. Suitable for a variety of fitness levels and abilities.
1-hour sessions twice a week for 10 weeks
Healthy Moves for Aging Well℠
Receive personalized, one-on-one coaching in the comfort of your home from a trained Healthy Moves coach.
Your coach will complete an initial fitness and fall risk assessment, teach you simple exercises tailored to your abilities, and provide follow-up coaching calls over three months to help you stay motivated and on track.
Simple exercises, such as arm curls, stepping in place, and ankle flexes, can make a meaningful difference. They can improve your strength, flexibility, balance, and everyday movement. You’ll feel more confident, comfortably manage daily activities, and continue doing the things you enjoy.
Home-based program with coaching and follow-up support over 3 months
Enhance Fitness
Stay active with a low-impact exercise program designed for all fitness levels.
Improve your strength, balance, flexibility, and endurance through stretching, low-impact aerobics, and strength-training exercises, helping you stay energized and maintain your independence.
Sessions three times a week for 8 weeks.
Arthritis Foundation Exercise Program
A gentle exercise program for people with arthritis or other joint conditions. Improve flexibility, range of motion, and muscle strength in a supportive setting.
1-hour sessions twice a week for 6 weeks
Physical Activity Programs
Staying active is one of the best ways to protect your health, maintain your independence, and continue doing the things you enjoy.
Designed specifically for older adults, our evidence-based physical activity programs help improve strength, balance, flexibility, and overall well-being while providing opportunities to connect with others and stay socially engaged.
Program are available in English and Spanish. Workshop length and format vary by program.

Join Bingocize online from the comfort of your home. This fun, interactive program combines bingo, fall prevention exercises, and health education, allowing you to stay active, connect with others, and improve your balance coordination and, mobility.
1-hour sessions twice a week for 10 weeks

Enjoy the benefits of gentle Tai Chi designed specifically for older adults with arthritis and other joint conditions. Improve your balance, flexibility, strength, and confidence while supporting both your mind and body.

Walk With Ease
If you can comfortably walk for at least 10 minutes, this guided walking program can help you build strength and stamina, reduce pain, and gain the confidence to stay active. You’ll also improve flexibility while learning to walk safely and comfortably.
Participants walk three times a week for 6 weeks

Advancing Healthy Aging Across California
Through federal grant funding, Partners in Care Foundation established the California Healthier Living Coalition, a statewide network recognized by the National Council on Aging for advancing healthy aging, chronic disease self-management, physical activity, and falls prevention.
The coalition supports organizations across California by sharing best practices, coordinating resources, and expanding access to evidence-based wellness programs.
