Yale School of Medicine receives grant funding to implement HomeMeds

Yale School of Medicine has received grant funding to implement HomeMeds, an evidence-based, in-home, medication review and intervention program developed and licensed by Partners in Care Foundation. The implementation of HomeMeds is part of a three year research project being carried out at Yale aimed at reducing healthcare utilization, particularly falls prevention. Yale will work with the University of Connecticut School of Pharmacy to conduct the medication reviews.
