Weingart Foundation Grant to Strengthen Partners in Care Foundation Operating Capacity

SAN FERNANDO, Calif. (Nov. 20, 2018)Partners in Care Foundation (Partners) announced today The Weingart Foundation (Weingart) recently awarded Partners a $200,000 grant in unrestricted operating support. This two-year grant recognizes Partners’ growth and commitment to high need communities of concern to Weingart.

Given recent changes in the California’s approach to Medicaid by increasing responsibility for long-term services and supports, the Weingart grant is particularly timely as Partners expands its services to Medi-Cal populations through new partnerships and more structured referral criteria. Integration of community solutions to social determinants of health with health care systems is a vital needed change.

Karen Schneider, Partners’ Vice President of Development, reflected, “Weingart’s generous support will help us to continue creating and adapting community-based social service and wellness programs for high-risk populations while leveraging emerging trends in social service and healthcare delivery.”
