Service Story: Helping to Reconnect a Family Through Accessible Transportation and More!

72-year-old Sharon lives with multiple health conditions and has severe depression. She relies on a walker and wheelchair to move around, which has limited her ability to travel comfortably outside of her home. She has missed multiple doctor’s appointments because of a lack of transportation and has not seen her family in over two years.

Sharon’s neighbor, who regularly checks up on her, referred her to Partners’ Multipurpose Senior Services Program (MSSP), a program for Medi-Cal eligible individuals who are 65 years or older, and in need of care management services. Participants tend to require nursing facility level of care, but with MSSP services and support, can live at home safely and independently.

Living with depression, it was difficult to do basic activities of daily living, like eating, brushing her teeth, bathing, and cleaning around the house. Sharon’s MSSP care manager set her up with chores and personal care assistance and connected her with a meal delivery service to ensure she received nutritious meals to regain her appetite. They also helped her enroll in Easy Lift Transportation, which provided her with wheelchair accessible transportation. This helped her make scheduled doctors’ appointments more frequently and visit friends or family if she wishes.

Her care manager even helped Sharon reconnect with her children through social media. Sharon expressed her gratitude and shared that she recently utilized the Easy Lift Transportation service to meet up with her son and grandson for the first time.

Sharon reflects, “MSSP helped me through one of the darkest times of my life, and for that, I will be forever grateful.”
