Partners in Care Foundation Recruits Eric Hazzard to Lead Community Care Hub

Sr. Director, Partners’ Community Care Hub

San Fernando, CA — Eric Hazzard, Ph.D. has joined the San Fernando-based Partners in Care Foundation (Partners) as a Senior Director for the agency’s Community Care Hub. In this role, he will bring social care solutions to health care partnerships through a new system extending medical and social services into the home and community, supporting those most in need.

This effort includes recruiting community-based social service organizations to join the Community Care Hub’s provider network and managing ongoing relations through the Hub. 

Partners’ President and CEO June Simmons noted “Eric joins us with a great background and personality well-suited to Partners’ mission. We were fortunate to work with Eric while he was in his last position and saw up close the strong values and people skills he brings to our agency.”

Before joining the Partners, Hazzard served as Strategic Sales Director in California and Hawaii for Unite Us. Before that he worked with TupeloLife Services as Chief Scientific Officer.
