Partners in Care Foundation Awarded NCQA Accreditation

The high quality of Partners in Care’s community-based care management has been recognized with a two-year Accreditation from the prestigious National Committee for Quality Assurance (NCQA). Partners is one of the agencies that has championed the concept of community-based care management organizations seeking quality Accreditation, and is only the second CBO in the nation to receive the NCQA seal.

Widely regarded as representing the gold standard for the assessment of and reporting on healthcare provision, NCQA is a private, non-profit organization dedicated to improving health care quality.

Partners received NCQA Accreditation for case management complex care, as demonstrated by the long-running and highly successful, Multipurpose Senior Services Program (MSSP), which provides community-based care management and evidence-based self-management programs, aimed at delaying or even preventing unnecessary nursing home placement. Data shows that Long-term Services and Supports (LTSS) have kept nursing home eligible seniors at home for a fraction of the cost of nursing home placement.

Responding to news of the Accreditation, Aloyce Rachal, Senior Director, MSSP said, “We are immensely proud that our work has been recognized. By providing a thorough, in-home evaluation of a person’s circumstances, we can enable the delivery of integrated services to best suit their needs. Often times, a few simple aids like a wheelchair ramp or grab bars, can prevent the accidents and falls that commonly lead to nursing home placement or premature death. A shocking 40% of seniors who suffer hip fractures die within one year.”
