Life ElderCare and SSPTV Integrate HomeMeds with Fall Prevention Program

Life ElderCare, an organization providing community-based services and supports to seniors in Alameda County, California, has coordinated efforts with Partners in Care Foundation to integrate the HomeMeds Medication Safety Program with Life ElderCare’s Fall Prevention Program. The combined programs are designed to increase participant’s strength, balance and flexibility and reduce the risk of falls, through the implementation of a medication review, home safety check and personal fitness trainer.

Under Life Eldercare’s HomeMeds license, Senior Support Programs of Alameda County’s Tri-Valley (SSPTV) will also integrate the HomeMeds Medication Safety Program with their current In-Home Exercise and Nutrition program. SSPTV is an independent, non-profit agency-serving seniors aged 60 plus in the cities of Pleasanton, Livermore, Dublin, and Sunol.
