L.A. Care contracts with Partners at Home Network for Complex Social Services

L.A. Care has contracted with Partners in Care Foundation to provide Complex Social Services, which supports community living for L.A. Care members deemed as high risk and high cost utilizers. L.A. Care is the largest health plan servicing the needs of dually eligible and low income clients in LA County. The contract will utilize the geographic coverage provided by the Partners at Home Network (PAH), a state-wide affiliation of community-based organizations that address the vital drivers of health outcomes, including issues responsible for preventable emergency department use, hospitalizations, and nursing home admissions.
