LA Care Awards Partners $5,000 to Combat and Manage Diabetes in the Greater San Fernando Valley Area

San Fernando, Calif. (July. 20, 2018)— LA Care Health Plan recently awarded Partners in Care Foundation (Partners) a $5,000 grant. The funding from LA Care’s Regional Consumer Advisory Committee enables Partners to improve diabetes prevention and management among the Greater San Fernando Valley area.

The goal of Partners and the LA Care grant are to increase access to diabetes prevention and management services for vulnerable and low-income residents through programs and workshops; raise awareness of local community services and resources focused on improving diabetes prevention and management; and engage local multiple sectors (healthcare, economic, transportation, faith-based, etc.).

“Our diabetic management health education programs are vital because they help those with high-risk diabetic health factors manage their disease,” said Partners CEO, June Simmons. “This grant helps us combine our unique approach to combat the disease.”

Workshops will be held at West Valley Mental Health Center in Chatsworth on Wednesday, August 1, 2018 from 9:30 a.m. to 11:30 a.m.; and at L.A. Care Family Resource Center in Pacoima on Monday, August 6th from 11:30 a.m. – 1:30 p.m. Space is limited. To participate, call (818) 837-3775 extension 112.
