The Community Wellness Department at Partners in Care Foundation helps older adults in our community stay healthy, mentally engaged, and entertained! Our team provides

evidence-based health promotion programs across the City and County of Los Angeles, and technical assistance for these programs across California.

As a Project Associate on our team, you will provide administrative support to the Program Director and Supervisor, City & County IIID Programs. An important part of the job is to provide leadership to reach overall objectives, within budget, to increase awareness and accessibility of evidence-based programs in Los Angeles and to track the team progress towards goals and objectives of the two grants.

Responsibilities:      

  • With the Program Director and Supervisor IIID, engage in strategic planning and policy analysis to identify potential partners, referral networks and funding sources to sustain ongoing evidence-based dissemination of programs in Los Angeles City and County.
  • Inform and monitor project activities, work plans, protocols, policies & procedures to ensure desired outcomes are achieved and surpassed, and report variances to the Program Director and Supervisor IIID so that corrections can be implemented.
  • Tracks trends, analyzes emerging issues, and initiates dialogue to determine the appropriate response to community needs, both internally and externally.
  • Assist in the development of strategy to meet the goals and objectives of the City and County IIID contracts and build collaborative relationships with colleagues in the field, volunteers, and community members.
  • Bilingual skills a plus.
  • Liaise with the ACL LA County Project Associate to implement, track and report on goals and objectives required by the ACL grants.
  • Oversee LA City and County reporting on HomeMeds results and liaise results with the ACL LA County Project Associate.
  • Serve as a liaison, ambassador, and advocate for Community Wellness’ evidence-based programs with public health, physician, health care professional, and payer communities
  • Respond to inquiries from prospective organizations to explore possibilities for partnerships, participation, and readiness to engage in evidence-based program delivery.
  • Assist Program Director and Supervisor with organizing leader/coach trainings and support leaders/coaches in implementing evidence-based programs
  • Assist the Program Director and Supervisor with scheduling evidence-based workshops in the City and County of Los Angeles
  • Report monthly deliverables in CTS and GetCare systems.
  • Assist with the development of the County IIID semi-annual and final reports.
  • Assist the Program Director with writing the CDBG Technical Assistance Evidence-Based Bulletin tip sheet, which is due by the last day of each quarter.
  • Assist the Program Director on CDBG-related technical assistance reporting due the 5th day after the end of each quarter.
  • Assist the Program Director and Supervisor IIID in the development of publicity and marketing materials for evidence-based programs
  • With the Program Director and Supervisor IIID, develop Policies & Procedures required by the Community Wellness Department’s IIID contracts. This person will be responsible for updating and sharing Policies and Procedures with Community Wellness staff at regular staff meetings.
  • Develop reports and provide ad hoc reporting from the Community Wellness Salesforce database
  • With Community Wellness leadership, oversee evidence-based program fidelity and assist the ACL Project Associate with maintaining affiliate agreements, and LA City and County affiliate oversite
  • Facilitate evidence-based programs according to departmental needs
  • Ensure compliance with all applicable laws and regulations, including those governing privacy and data security

Minimum Qualifications:

  1. Enthusiastic about working in a team environment to improve the lives of older adults and adults with disabilities.
  2. Completion of a Bachelor’s degree in a related discipline (e.g., Public Health, Social Work, Healthcare Administration) and at least 2 years of directly related experience; or an equivalent combination of training and experience related to the duties of the position.
  3. Excellent interpersonal skills; ability to work well with all styles. Must be a team player, have demonstrated patience, humility, and flexibility.
  4. Highly motivated and creative, able to work independently and be self-directed requiring minimal supervision; must be organized and demonstrate excellent follow-through.
  5. Possess strong written, verbal, and interpersonal skills, attention to detail and accuracy, and the ability to think creatively and generate new ideas.
  6. Proficient in Microsoft Office computer applications (Word, Excel, PowerPoint, Access etc.).
  7. Salesforce experience preferred.
  8. Possess a valid California driver’s license and proof of auto insurance.
