Job Title: Hospital Navigator

Program: Enhanced Care Management

Partners in Care Foundation is growing and we are looking for a Hospital Navigator to join our ECM team!


  • Two years of direct case management experience or related hospital, nursing, or public health experience with medically fragile, and/or special needs populations in the field.
  • Bachelor’s degree in health and human services (public health, nursing, social work, psychology, sociology, etc.) preferred.
  • Valid California driver’s license, reliable transportation, and proof of current liability insurance required.
  • Ability to work effectively from home daily (workspace, stable internet connection, etc.).
  • Experience with Microsoft Office Suite (Excel, Word & Outlook) preferred.

Care Coordinator Responsibilities:

  1. Making regular contact in the community (member’s home, hospital, shelter, etc.) with all assigned members.
  2. Confirming member’s eligibility by verifying qualifying chronic condition with member.
  3. Completing a Health Risk Assessment (HRA) with member including identifying member’s Primary Health Concern (PHC) and other needs on a regular basis.
  4. Creating a Health Action Plan (HAP) with each member and regularly updating HAP according to member’s ongoing needs and service utilization.
  5. Supporting member’s access to health care plan services and resources.
  6. Coordinating and monitoring ongoing referrals to community resources and services.
  7. Documenting all meetings and services with member.
  8. Attending trainings and weekly supervision and meetings as scheduled.

Program Coordinator/Hospital Navigator Duties:

  1. Monitoring Collective Medical and reporting member admissions to Care Coordinators.
  2. Participating in interdisciplinary care team (ICT) meetings to present Collective Medical findings and inform the discussion regarding health and public health perspectives.
  3. Building relationships with hospital and emergency department staff at area hospitals.
  4. Identifying and coordinating medically oriented training opportunities.
  5. Coordinating meetings and conducting office and field activities to support HHP Team in regard to services with medical high acuity HHP members.

Physical Demands:

  • Daily driving to visit clients in homes, shelters and other areas and locations.
  • Lifting capability of 5-10 pounds, depending on what is taken in the field (laptop, paperwork, etc.).
