Providing Low Income Older Adults with Minor Home Modifications to Age in Place
The Older Adults Home Modification program (OAHMP) enables low-income older adults to remain in their homes through an assessment by an Occupational Therapist or a Certified Aging-in-Place Specialist and low-cost, high-impact home modifications to help prevent falls related injuries, improve accessibility, and promote independence.
The goal is to empower low-income, high need older adults to “age in place”—to live safely and comfortably in their own homes, rather than move into nursing homes or assisted living facilities.

Who Should Apply?
- You own or rent your primary residence in Los Angeles County where the home modifications will be installed.
- You or someone in your household (applicant) is 62 years of age or older.
- Your total household income based on family size is below the maximum limit listed below.
- You or someone in your household (applicant) requires home modifications to prevent falls and avoid the risk of having to move to an assisted living facility due to safety concerns in the home.
Income Guidelines

To qualify, your total household adjusted gross income (IRS 1040 AGI) must be below the following limits.

Types of Modifications May Include:
- Grab bars and handrails
- Lever-handled doorknobs and faucets
- Handheld shower head
- Tub cuts
- Non-slip strips for tub/shower or stairs
- Improved lighting
- Temporary ramps, non-permanent graded ground ramps, or stair railings

This program is made possible through grant funding from the U.S. Department of Housing and Urban Development. Funding is limited and available only during the designated grant period, from 2024 to 2027. While we are committed to supporting as many eligible households as possible, please note that submitting an interest form does not guarantee the receipt of services, as funding is capped and subject to availability.
