About Partners in Care Foundation

Partners in Care Foundation is a recognized leader in care coordination and community-based health services. Since 1997, we have worked to bridge the gap between medical care and community-based services, the non-medical support people need to live safely, independently, and with dignity in their own homes.

We develop and deliver scalable, practical solutions that address the everyday challenges impacting health and recovery. Our work targets the non-medical drivers of health—such as food insecurity, housing instability, medication management, transportation barriers, and home safety risks—that can lead to poor outcomes and unnecessary hospital visits.

Through a network of community-based partners and our experienced staff, we coordinate and provide essential services that help individuals with complex health needs remain in their homes and communities. This approach helps prevent costly hospitalizations and reduces the need for premature nursing home placement.

At the heart of our work are the deep, personal connections we build with the people we serve. These relationships drive real, measurable improvements in health outcomes and deliver proven cost savings for healthcare systems.
