Waiver Care Management Supervisor

Reports To: Senior Director, HCBA
FLSA Status: This position is salaried and exempt
Salary: $68,640-$83,000

Qualifications

Requires Masters’ degree in Social Work.

Requires two years of experience supervising care management staff and three years of experience working with seriously ill, frail, or disabled adults.

Responsibilities

  • Responsible for planning, organizing and developing the principles and techniques employed in a comprehensive case management model for frail and disabled adults.
  • Ensures POT is person-centered and includes the individual’s goals and preferences.
  • Review and approval of all client service plans, and client cost monitoring.
  • Supervises social work care managers, nurse care managers, care management aide and case management support staff including approval of timecards and performance management.
  • Analyzes social service/health operations internal and external to the program and recommending changes to improve service accessibility for clients.
