Thank you for your interest in applying for the Partners in Care Foundation’s HUD Older Adult Home Modification Program.

What to Expect Once Enrolled in OAHMP?

  • Survey & Assessment
    We’ll contact you by phone or email to complete a brief survey, helping us prioritize applicants with the most urgent safety needs.
  • Home Visit
    A licensed occupational therapist  or a Certified Aging-in-Place Specialist will schedule an in-home assessment.
  • Customized Plan
    Based on the assessment, our coordinator will create a work order tailored to your goals and your home’s needs.
  • Installation & Modifications
    We’ll coordinate with a licensed contractor to complete the work, ensuring everything is done safely and professionally.
  • Final Visit
    The occupational therapist or the Certified Aging-in-Place Specialist will schedule the final in-home assessment to inspect the completed work and to ensure the proper use of any new equipment.

This program is made possible through grant funding from the U.S. Department of Housing and Urban Development. Funding is limited and available only during the designated grant period, from 2024 to 2027. While we are committed to supporting as many eligible households as possible, please note that submitting an interest form does not guarantee the receipt of services, as funding is capped and subject to availability.
