Care Transitions for People with Dementia. There are several care transition programs to reduce unnecessary hospital re-admissions, but most are not tailored to meet the needs of people with mid-stage dementia and their family caregivers. This presentation focuses on a project that was developed to create tools and training curriculum that specifically engaged caregivers and taught care transition coaches practice-based approaches to reduce 30-day re-admissions.
