Chronic Disease Self-Management
Healthier Living
Teaches physical activity, nutritious eating and relaxation techniques that help those living with chronic conditions. Supports self-management of ongoing health conditions such as arthritis, heart disease, diabetes, high blood pressure, lung disease, and cancer. Sessions take place weekly for 6 weeks. The curriculum has also been culturally translated and is offered in Spanish as Tomando Control de su Salud.
Chronic Pain Self-Management
For individuals dealing with ongoing pain. This workshop focuses on strategies to cope with and reduce chronic pain including relaxation, gentle movement exercises, and improving communication. Sessions take place weekly for 6 weeks and are available in both English and Spanish.
Diabetes Self-Management
This workshop helps individuals understand their blood sugar level, manage stress, and make their diet work. Sessions take place weekly for 6 weeks and are available in both English and Spanish.
Fall Prevention and Physical Activity
A Matter of Balance
This program is for people who have fallen in the past, have a risk of falling or who may be reducing their activities due to a fear of falling. Participants learn to change their environment to reduce fall risk factors, to view falls and the fear of falling as controllable, set realistic goals to increase activity and exercise to increase strength and balance. Sessions are 2 hours, once per week for 8 weeks or twice per week for 4 weeks.
Bingocize
This program is a 10-week program combining Bingo with fall prevention exercises and health education. Bingo games are twice per week for 1 hour.
Tai Chi for Arthritis
This workshop meets for 10 weeks, twice per week for 1 hour and teaches core movements in the Tai Chi Sun Style and the Tai Chi principles that build balance of mind and body.
Enhance Fitness
This exercise program helps participants at all levels of fitness become more active, energized, and empowered to sustain independent lives. The workshop focuses on stretching, flexibility, balance, low-impact aerobics, and strength-training exercises. Sessions take place 3 times per week for 8 weeks.
Arthritis Foundation Exercise Program
Low-impact exercises and gentle activities help individuals increase joint flexibility, range of motion, and maintain muscle strength. Sessions take place for 1 hour twice per week for 6 weeks.
Walk With Ease
If you can be on your feet for 10 minutes without increased pain, you can have success with Walk With Ease! This program will help you motivate yourself to get in great shape, walk safely and comfortably, improve your flexibility, strength, and stamina, and reduce pain. Participants walk three times a week for six weeks.
