Congratulations to the Partners in Care Foundation Network Team! For the third consecutive quarter, our Engagement Center is rated #1 in the nation for Anthem MediCaid Engagement for Anthem’s Community Care Coordination Program (A3C). Engagement rate is dependent on the number of viable referrals who completed a comprehensive assessment to identify social needs and gaps in care. Our Network team then coordinates services and links members to resources to meet those need and address the gaps.
