Partners at Home Network Continues Statewide Expansion

Partners at Home Network (Network), a network of community-based organizations (CBO) led by Partners in Care Foundation, continues to expand rapidly across California with the addition of five new members. Meals on Wheels & Senior Outreach Services in Contra Costa County, Napa Solano Area Agency on Aging, St. Agnes Medical Center in Fresno, San Diego Aging and Independence Services, and LIFE ElderCare in Alameda County joins Partners in Care and 12 other CBO’s providing coordinated social services to vulnerable people across the state.

The Network addresses the social determinants of health, including issues that, left unchecked, frequently lead to unnecessary emergency department use, hospitalizations, and nursing home admissions. The Network streamlines access to multiple community-based care “extenders,” providing hospitals, accountable care organizations, physician groups and health plans with a simple, efficient, coordinated community care system, staffed with a well-trained, culturally and linguistically competent workforce, experienced in helping patients with complex, health-related social needs.

“The Network provides an easy, ‘one call does it all’ solution for healthcare providers, connecting multiple CBO’s across the state to deliver comprehensive, continuous, quality care to older adults,” explains Sandy Atkins, Vice President, Strategic Initiatives, at Partners in Care. “The Network offers three key service lines: Care Management, including post-discharge care transitions; Long-Term Services and Supports; and Evidence-Based Health Self-Management, including Falls Prevention, Chronic Disease Self-Management, and Diabetes Self-Management workshops.”
