If you, or someone you care for has recently been hospitalized, or is suffering from a chronic condition such as diabetes or arthritis, or has a disability that requires ongoing care, you’ll know that it can often feel like health issues are taking over your life. The good news is that it doesn’t have to be this way. There are things you can do to reduce your risk of hospital readmission, lessen symptoms of chronic conditions, improve your quality of life and take control of your health, rather than letting it take control of you.

Health Self-Management Workshops, delivered in community settings across Los Angeles, equip you with the skills and confidence you need to manage your chronic condition and maintain independence. Led by trainers experiencing the same chronic condition as you, these workshops have been carefully evaluated to ensure they deliver skills that can make a genuine difference to your life. Participant feedback has been extremely positive.

If you or a loved one is 65+, on Medi-Cal, and physical or other disabilities are putting you at risk for nursing home placement, our Life Is Better At Home℠ Multipurpose Senior Services Program (MSSP) can arrange for services that assist in daily living and help keep you safely in your home.
