(Oct. 24, 2017)— U.S. Administration for Community Living (ACL) awarded Partners In Care Foundation (Partners) an $850,000 federal grant to help reduce serious risk factors associated with chronic pain or diabetes through Chronic Disease Self-Management Education (CDSME) programs. This grant enables Partners to provide evidence based programs that reduce serious risks related to diabetes and chronic pain in areas with high-incidences of preventable suffering. Sara, 89, was recently discharged from hospital and subsequently a rehabilitation facility following a fall in her apartment which had resulted in facial fractures and broken ribs. Lucy, Sara’s daughter, was present during the visit with coach Traci. Although Sara would like to remain in her apartment, she now relies on a walker to get around and there is no elevator. Following a cancer diagnosis, Helen, 68, a former education counselor at the University of Arkansas, moved to California to be closer to her son and daughter.
