Q. What are the 2008 deadlines for the Large Grant Proposal?

Q. Will grant requests less than $500 be considered?
A. Family Care Network will consider grant requests less than $500. An emphasis is placed on serving those with more urgent needs, needs that meet our core guidelines, are under $500 and can not be considered in or wait until our Large Grant cycle deadlines. Q. Do I need to have more than one vendor quote?
A. Yes, two vendor quotes are required for our Large Grant application. Quotes need to be less than 90 days old and competitive in pricing. However, only one quote is required if you choose a Change A Life Foundation preferred vendor.

Q. Who can qualify for a grant?
A. The program’s purpose is to significantly help individuals and families who, through no fault of their own, have experienced an injury, illness, disability, or catastrophic life event and are suffering financial hardship, by providing support for critically needed resources promoting self-sufficiency and improved quality of life. Priority is given to clients who do not have ability to pay or obtain resources on their own. Applicants must meet program guidelines for low-income. The program intends that funds will be used as a last resort request. Applications must discuss research into all available alternate funding sources. All of these elements are required for applications to be eligible.

Q. Where can I find out the income limits?
A. All applicants must meet the low-income threshold of Department of Housing and Urban Development income limits. The following chart displays the limits:

Q. Will Family Care Network consider applications for the purchase of modified, new and/or used vehicles?
A. Family Care Network will not consider applications for the purchase of a modified, new or used vehicle. However, applications will be considered to modify a existing vehicle owned by the applicant or his or her family. The applicant or applicant’s immediate family member must own vehicle. The vehicle must be in good condition with mileage less than 100,000.

Q. Will Family Care Network consider applications for Academic Scholarship Assistance?
A. Every individual deserves the chance to attend college and gain the amazing experiences that come along with pursuing higher-education. Change a Life Foundation seeks to make this possible for students who can not afford to pay for the cost of college themselves. Students who receive these scholarships are in serious need of financial help, and have excelled in their high school education. The Foundation's granting is strategic and focused. Scholarships are only provided to graduating high school seniors in the following School Districts: Anaheim Union High School District, Santa Ana School District, Los Angeles Unified School District, and the San Diego High School District. Q. Will Family Care Network consider applications for rent or mortgage assistance?
A. Family Care Network will not cover the cost of rent except in the following circumstances:

  • Client or Family fleeing domestic violence
  • Elderly client being priced out or evicted
  • Emancipating or Transitioning Foster Youth

Q. I need new furniture; will Family Care Network consider an application for purchase?
A. Family Care Network does not purchase furniture or appliances unless the appliance or the lack thereof presents a severe safety and/or health risk. For example, grants have been awarded to purchase refrigerators to safely store insulin and air conditioners for individuals with respiratory illnesses.

Q. What are the guidelines for submission if I need dental treatment?
A. Guidelines for dental treatment include:

  • Minor dental repair is considered if the request is $2,000 or less;
  • All other resources must be explored prior to submission;
  • More extensive dental work such as dentures can be considered but cannot exceed $5,000;
  • Requests for implants or veneers can not be considered; and
  • Orthodontia for foster care youth, 11 years of age or older who are experiencing pain, disfigurement or other health issues

Q. What are the guidelines for funding a grant for a computer?
A. The applicant must desperately need the computer for self-sufficiency. The applicant must demonstrate and guarantee success through receipt of a computer. We can consider a request for a computer that will be used as an adaptive communication device and will create a real and measurable life change. All other resources must be explored prior to submission.

Q. What if I need critical modifications for easier access in and out of my home?
A. Individuals who apply for a home modification (i.e. wheelchair ramp or shower modification) must own the home. Repairs and modifications will not be done on rental property. If the applicant or family owns the home that the applicant is living in, the following must be explored:

  • Line of Equity/Home Equity Loan
  • Second/Reverse Mortgage
  • Refinance

Q. How long after my grant is submitted will I find out if I have been approved?
A. It takes about between 5 and 7 weeks for Change a Life Foundation to notify Family Care Network if a grant has been approved or denied. If awarded, it will take 1 to 2 weeks for payment to be delivered to vendor.

Q. Once the grant is approved, is the check made payable to me?
A. Checks are not made payable to any individual. It is made payable to the vendor.
