Family Care Network - Applications

Becuase of the generosity of QueensCare and Pfaffinger Foundation, Immediate Needs Grants are available for amounts up to $500 (larger amounts can be considered) and are generally used for urgent needs, those resources that will improve quality of life and prevent premature hospitalization. Applications are accepted ongoing. Please note residency requirements for our Immediate Needs Grants (Specific Zip Codes in Los Angeles County)

Hearing Aids
Minor Dental Treatment
Medical Equipment and Supplies
Vision Screening and Glasses
Inpatient and Outpatient Healthcare

Immediate Needs Fund Application

Large Grant Funding is provided by Change a Life Foundation - grants range from $500 to $7,500 and cover a wide range of critical health and social service resources unreimbursable by any other source
