We are here to serve as a catalyst to shape a new vision of care. There’s a good reason why “Partners” is the first word in our name.

We live up to that concept by partnering with organizations, families, community leaders in the work of changing healthcare systems, changing communities and changing lives.

  • Community Partners
    Hospitals and health systems, universities, social service agencies, and public organizations engage with Partners in finding solutions to pressing healthcare and social service needs.

    The nature and content of these partnerships are as varied as the partners are, but with the common goal of delivering services more effectively and efficiently for those they serve.

  • Funding Partners
    Partners helps funders realize their goals of effecting an agenda for change, from unrealized ideas of improvement to evidence-based, sustainable realities and then to dissemination and replication of proven models locally and nationwide.

Through Partners' Institute for Change (IFC)/Research Center, fundable concepts are designed, tested, and evaluated as to their effectiveness. We speed change through dissemination of proven models of care to bring more efficient health and social services to diverse populations and communities.

The Institute for Change consults with and advises many local, national and regional organizations in creating systemic change. The IFC Principal Investigators and Co-Principal Investigators have published major academic articles and books, and presented their findings at professional meetings all over the country.
