Family Care Network
Partners in Care launched Family Care Network in 1997 as a safety-net program for low income, vulnerable individuals and families facing a disability, chronic illness or catastrophic life event. Because sudden injury or illness can challenge or even overwhelm families, Family Care Network enables individuals and families to overcome unexpected obstacles to independence, dignity and a life of full potential by arranging for critical health and social service resources. Since 2001, Family Care Network has arranged for nearly $2.0 million in resources vulnerable families so desperately need but cannot afford. This 'direct rescue' is made possible by the generosity of Change a Life Foundation.
An emphasis is placed on critical health and social service resources that prevent falls and other health concerns, costly hospitalization and premature nursing home placement. Our resources increase self-sufficiency, improve quality of life and preserve families during very difficult times.

Family Care Network collaborates with a network of care management partners such as hospitals, rehabilitation service providers, senior centers, legal advocacy centers and children and youth service agencies. Together, we identify and screen eligible clients, develop individual service plans, apply for funding, assure that funded services are delivered and evaluate program impact.
Family Care Network welcomes social workers, service coordinators, discharge planners, psychologists, attorneys and all other advocates who represent clients with unmet health care and social service needs.
