Our Mission:
Santa Clarita Adult Day Health Care Center helps delay or prevent premature placement in a nursing home, psychiatric facility or other institution for the frail elderly and other physically or mentally impaired adults 18 and over.
Our therapeutic environment is designed to promote socialization and stimulation and maximize individual potential for independence, dignity and quality of life among the elderly and other functionally impaired adults.
Our Center’s Benefits: When Help is Needed Caring for a Loved One
Many families feel they have only two options for care: Either someone in the family takes on the role of full-time caregiver, or the family places the loved one in a nursing home.
Santa Clarita Adult Day Health Care represents the third option. By providing quality care for your loved ones in an enriching and safe environment, we allow caregivers to continue caring for their loved one at home while meeting other professional and family obligations.
Our Adult Day Health Care is well suited for those who have suffered a stroke, traumatic brain injuries and other physical, mental or memory impairments.
Our Models of Care:
Santa Clarita Adult Day Health Care incorporates two models of care:
The Health Model
The Social Model
Our Center’s Staff:
Santa Clarita Adult Day Health Care provides quality care at an affordable cost. Our program is staffed by a multi-disciplinary team of nursing and social work professionals who evaluate each participant and customize their individual plan of care.
