Supporting Frail Seniors and Their Families in the Home

Partners in Care Multipurpose Senior Services Program (MSSP), a Medicaid/Medi-Cal Waiver program, helps prevent vulnerable older adults from premature hospitalization and nursing home placement.

We provide case management services and arrange in-home services to assist clients with daily living needs and teach them how to lead healthier and more active lives using powerful approaches to prevent decline, avoid unnecessary hospitalizations ornursing home placement and improve overall health.

The MSSP program serves medically isolated, frail and underinsured individuals who are at-risk of family breakdown, premature institutionalization or loss of independence. Our program targets adults 65 years of age or older in Southern California, including low-income, diverse urban areas, as well as isolated and rural segments of Los Angeles County.

Program qualifications

  • 65 years of age or older
  • MediCal recipient with no share of cost
  • A willingness to participate in the care plan by allowing services to be provided in your home
  • Eligible for placement in a nursing facility but you wish to remain in your home

MSSP collaborates with caregivers and clients to develop comprehensive care plans, obtain critical resources and services and ensure participation in essential programs that will ultimately assist the client in his or her daily living, provide a higher quality of life and ensure access to care.

MSSP care managers are nurses and social workers who visit the home and conduct a health and psychosocial assessment. Together they evaluate individual needs to determine the resources that would benefit the client and/or caregiver and foster self-sufficiency.

Our Client Population


If you or a loved one is a MediCal recipient and physical or cognitive disabilities are increasing the risk for nursing home placement, MSSP can arrange or pay for services to ensure a safe and comfortable life at home. The focus of our MSSP program is the provision of services and referrals to delay or prevent premature placement.
