Healthy living and improved quality of life for those we serve are the primary goals of Partners' collaborative projects and programs. We develop, implement and evaluate programs for older adults, the uninsured, families and children. Partners believes that early disease identification and intervention among thse groups alleviates suffering and minimizes the future need for costly care. Providing supportive services at home avoids nursing facility placement, hospitalization and mitigates the risk of chronic disease. Expanded access to basic care and self-care disease management reduces the burden on our healthcare system. The evidence-based results are quality care and a wiser allocation of scarce health care resources.
