Family Care Partnership, through a network of community agencies and organizations working together, focuses on addressing the complex challenges faced by caregivers, veterans, seniors and those with disabilities. 

Building on the existing community service network, the Family Care Partnership is guiding the development of an integrated system of community-based long-term care to promote quality of life, expand choice, enhance family life, reduce caregiver burn-out and promote independence for the growing population of veterans, seniors and adults with disabilities--and those who care for them.

When fully implemented, the anticipated outcomes of this initiative include:

  • A working collaborative of community leaders and service providers is established to ensure that a comprehensive service network is in place and fully integrated.
  • Participating organizations are stable and are supported by a consistent flow of referrals through this integrated network.
  • The network of supportive resources is strengthened and clear points of entry into the network are created. Services are expanded and sustained locally through the new partnerships.
  • Enhanced community capacity ensures frail older and disabled adults have access to resources to support their remaining independence in the community longer.
  • Dignity and independence are preserved, family life quality is improved and cost to the community is reduced.
  • The public expense of paying for costly institutional care is reduced when systems are in place to support and sustain individual choice to remain in the community.
