Family Care Partnership
A collaborative led by Partners in Care

Family Care Partnership, through a network of community agencies and organizations working together, focuses on addressing the complex challenges faced by caregivers, veterans, seniors and adults with disabilities. 

Through the generous support of The SCAN Foundation's one year planning grant, it will lead to the creation of "Under One Roof", bringing together relevant service options and resources for these families and individuals, community agencies and the medical community.

Building on the existing community service network, the Family Care Partnership will guide development of an integrated system of community-based long term care to promote quality of life, expand choice, enhance family life, reduce caregiver burn-out and promote independence for the growing population of veterans, seniors and adults with disabilities--and those who care for them.

Multipurpose Senior Services Programs (MSSP) of Partners in Care of the San Fernando Valley and South Los Angeles 
Personal Assistance Services Council 
Bet Tzedek
Henry Mayo Newhall Memorial Hospital
North Los Angeles County Regional Center
Santa Clarita Adult Day Health Care Center

 

Once fully implemented, the anticipated outcomes of this initiative include the following:

  • A working collaborative of community leaders and service providers will be established to ensure that a comprehensive service network is in place and fully integrated going forward.
  • Stability of the participating organizations will be supported by a consistent flow of referrals through this integrated network.
  • The network of supportive resources for family caregivers, veterans, seniors and adults with disabilities will be strengthened and clear points of entry into the network will be created. Services will be expanded and sustained locally through the new partnerships.
  • Because of enhanced community capacity, frail older and disabled adults will be able to access resources to support their remaining independence in the community longer. Dignity and independence will be preserved, family life quality will be improved and cost to the community will be reduced.
  • Sustaining continued community living will reduce the public expense of paying for costly institutional care through supporting individual choice for community living.

When you become a caregiver, your world changes.

The order of things may be reversed. Maybe they used to take care of you. Now, you must take care of them. Home is not safe enough, but you believe away-from-home is not caring enough. When it comes to a loved one, money should be no object, yet there are some care giving challenges that all the money in the world cannot solve. The demands can be overwhelming, but you feel alone. The changes caregivers face may happen suddenly or develop gradually, yet forcefully, over time. Whatever the details, when you become a caregiver, your world no longer exists in the same recognizable form.

We may all become caregivers, if we are not already. Aging parents, spouses, disabled partners, injured or ill children, friends in need - caregiving is an equal-opportunity role, and major challenges exist no matter what your status in life.

Beyond its life-changing impact on individuals and families, caregiving is a community issue. So, Partners in Care (Partners) is joining with other community leaders to integrate the local safety net of services for older adults, the disabled and their families.
