scope of services and resources available to individuals with PD and
their families throughout Southern California to obtain the information,
support and services they need. This partnership sought to idenitfy and increase access to community resources and support services for Parkinson Disease (PD) patients and families served by The Center. |
Partners in Care Foundation (Partners) to serve as a collaborative partner in fulfilling a planning grant awarded from California Healthy Cities and Communities, a program of the Center for Civic Partnerships.The role of Partners was to develop, conduct and analyze a community survey to determine residents’ and community leaders’ needs, interests, expressed opinions and priorities on what activities, quality of life indicators and attributes would make San Fernando a Healthy City, including health-related programs and healthy lifestyle activities. |
Partners helps concept, find funding and lead the Antelope Valley Partnership for HealthWith the leadership of Partners in Care Foundation as one of its founding members, AVPH as a collaborative has taken a leadership role in improving the health and wellness of the Antelope Valley residents. AVPH has remained at the forefront of community response to health and safety needs, particularly to the population that is underserved, underprivileged, low-income uninsured residents and those living in rural outlying communities of the AV. |
Non-Profit Organization brings in Partners to Consult on Community Benefit ProgramA non-profit hospital that has been one of the longest standing and premier facilities in the Long Beach area and funded as a Disproportionate Share Hospital is home to a substantial immigrant community that comes mostly from Mexico, Cambodia, Laos and Vietnam. Median income for the area is below the national average and many neighborhoods are considered “underserved.” Services to this community include multilingual, multicultural social and education programs focusing on health promotion, disease prevention and healthcare access. Partners in Care Foundation was asked to provide strategic planning consultation with the specific goals of better understanding the program’s structure, clarifying future direction, identifying resource needs and options, and better understanding how the program and medical center might be more closely aligned in meeting mission and business goals. |
Partners in Care an Early Member of the Access to Care Collaborative Dedicated to Creating Timely Access to Care for the Working, Poor Uninsured in the San Fernando ValleyThe Access to Care Collaborative was formed in 2001 to create new access and continue to care for working uninsured adults in the SFV who have little or no access to care, who often suffer from chronic and debilitating disease. Early detection, treatment and coaching for self-care can preserve precious healthcare resources. By diagnosing and treating these diseases, especially diabetes, early and collaborating with partners who share a like mission, both suffering and costs will be mitigated. Through hospital leadership, forming collaborative partnerships with other community clinics and safety-net providers, the Access to Care Collaborative (ACC) was established in 2001 in response to the uninsured crisis in Los Angeles. Strategic planning developed a model of expanding the capacity of current community and free medical clinics in the San Fernando Valle by recruiting and coordinating volunteer physicians, nurses, and other healthcare and lay professionals. |
