Although bed bound and living alone, the 84 year old retired dentist received frequent assistance from a caregiver and close neighbor for his declining medical condition and diagnoses of Alzheimer’s disease, prostate cancer, and hypertension.
Chemotherapy left him weak and worsened his condition. Increasing falls led him to the hospital. During a recent hospitalization, his physician became alarmed by 12 daily prescription medications and 15 daily supplements. A critical referral was made to the project consultant pharmacist.
Based on the recommendations of the pharmacist, his physician immediately discontinued the duplicated medication. The patient and caregiver were also educated about preventing falls and medication errors.
