27 Medications; Erratic Compliance

 

Living alone with minimal support and assistance, an 87 year-old widow taking 27 different medications was referred to The Medication Management program.

She was unable to maintain a stable medical condition and she frequently forgot to take this overwhelming amount of medication.

In addition to multiple hospitalizations for chest pains, shortness of breath and heart surgery, she also suffered from arthritis, congestive heart failure, glaucoma, diabetes, hypothyroidism and an extensive history of falls.

Following a critical screening and consultation, three medication-related problems were recognized as contributors to her frequent falls.  Review of her records by a consultant pharmacist led to the discontinuation of several dangerous medications and therby reducing the risk of a fall--a fall that could lead to swift decline in an elderly person.
