Partners in Care Foundation Receives Grant Award from the Jewish Community Foundation of Los Angeles in Support of "Healthier Living" Program for Adults

Partners in Care Foundation Receives Grant Award from the Jewish Community Foundation of Los Angeles in Support of “Healthier Living” Program for Older Adults

(San Fernando, CA, April 8, 2008) - Partners in Care Foundation has received generous support from the Jewish Community Foundation of Los Angeles to strengthen the expansion of the “Healthier Living: Managing Ongoing Health Conditions” program in Los Angeles County,” announced W. June Simmons, President and CEO of the San Fernando, California-based non-profit healthcare and social services organization.

“Healthier Living” is an evidence-based program that was developed and has proven effective through research conducted by Stanford University. Participants have demonstrated significant improvements in exercise, chronic disease management, communications with physicians, general health, and increased community participation and socialization,” noted Ms. Simmons. “This program also demonstrates that prevention is key, and catching risk factors early and identifying chronic conditions early all alert people to the need to care well for themselves and equips them to optimize their health and thus their quality of life.”

The “Healthier Living” curriculum includes behavior modification and coping strategies to enable the participants to manage chronic diseases and medications, and increase physical activity. After completing the workshops participants demonstrate significant improvements in their self-reported general health.

The grant award will facilitate Partners in Care’s efforts to introduce “Healthier Living” and its Spanish counterpart, “Tomando Control de su Salud” (Taking Control of your Health), to Healthcare Partners Medical Group which will enable 140 older adults to successfully complete the program. Additionally 4 Master Trainers and 20 Lay Leaders will be trained to provide a strong foundation within Healthcare Partners Medical Group for subsequent “Healthier Living” workshops and training.
