The acuity level of patients in assisted living facilities has steadily risen over the years. Ten years ago, these same members would have resided in Skilled Nursing Facilities. These members have difficulties getting to the clinics, multiple co-morbidities, and complicated medication regimens. They often use the emergency department and the hospital for their primary care. The ALF evaluation is testing how the improves patient access, clinical outcomes, financial outcomes and patient satisfaction.
The ALF program provides on-site primary care by a physician and nurse practitioner team. The model includes evaluation of medical problems, advance care planning; function, depression, fall risk, and cognitive status assessment on every patient. Visits are a mix of one-on-one visits and group visits. A total of 23 KP members were enrolled in the ALF program. Partners in care Foundation found trends of increased patient satisfaction at six months and a reduction in the use of emergency room and hospital.
