Be Well Nutrition and Exercise Program

Studies have documented benefits of physical activity & diet for older people with chronic illnesses. Evidence-based programs achieve powerful outcomes that can be disseminated through solid partnerships. This initiative provides a framework for developing a community-based partnership to implement a tested model that will improve at-risk KP members health and ability to manage their diseases. The BE WELL program is testing a model aimed at improving patients’ health and reducing hospitalizations. BE WELL provides nutritional counseling and low impact exercise and weight management especially designed for moderate to high risk, chronically ill seniors.

Partners in Care Foundation evaluated the impact of this program on several client-level outcomes. Among participants in the Be Well Nutrition education and exercise program significant reductions were seen in depression levels (p=.003), nutrition risk (p<.001), fear of falling (p<.001), number of falls (p<.001), injuries from falls (p<.001) and improved blood pressure systolic (p=.007) diastolic (p<001).
