This article discusses and analyzes a myriad of issues pertaining to medication management for home health patients. It reports on the prevalence of medication problems in this population, analyzes recent regulatory changes that address those problems, describes three model programs that aim to improve home health medication management, and discusses pending legislation that could increase the availability of pharmacists’ services for home health patients. |
